Once you've attained a complete friendship or rivalry and given him gifts, tackle the final-stage relationship quest Questioning Beliefs.
14 Feb 2016 I didn't realize in Dragon Age 2, you can “lock in" the friendship/rivalry once it's maxed out. (I had assumed it would continue to change even if you reached the limit of one or the other.) So I thought I would shorten the Fenris/Approval wiki page options to which ones will gain you friendship with Fenris
