Select the frame and from the Type list choose a form element type. Enter a name for the form field. Choose an event and add actions to associate with it.
To create editable PDF's like those you need to use Adobe Acrobat and its tools to create and edit forms. If you have Adobe InDesign CS6 or CC you can start your form in this software and finish it in Acrobat. There are several tutorials on Create and analyze forms, such as these, among others:
