In patients with infective endocarditis (IE), splenic involvement is a rare but well-known adverse event. The treatment of patients with IE and splenic abscesses is still challenging and controversial. We report 3 patients with IE and splenic abscesses who underwent successful valve replacement and splenectomy.
1 Oct 2016 Conclusion. We report a case of splenic abscess complicated by infective endocarditis. On the other hand, a high index of clinical suspicion of splenic abscess should be maintained while treating patients with infective endocarditis.
If valvular surgery is done first, prosthetic valve infections might occur because of the existence of SA. A guideline states that, if possible, splenectomy should be performed before valve replacement surgery, in order to mitigate the risk of infection as a result of the bacteremia from the abscess [5].
In case 2, the splenic abscess was diagnosed as ruptured; valve replacement was performed, followed by splenectomy through a separate skin incision.
