2 Dec 2012 Step 1: Launch the Find My iPhone app on a different iOS device. Alternatively, you can go to iCloud.com, login with your Apple ID, and click on Find My iPhone. Step 2: Select the iOS device that's stuck in Guided Access from the list in Find My iPhone. Next, tap on “Lost Mode" at the bottom of the screen.
Otherwise the iPad just goes right on into Guided Access. If you have iCloud Find my iPad configured, you could pretend it is lost and set a lock code to break out of this.. There is no way that I know of to bypass a forgotten Restrictions passcode. @peineke That is interesting, I will have to play with it. I'm pretty sure Casper

19 May 2017 Disable Guided Access If You Forgot Password on iPhone. In order to disable Guided access without entering the password, you need to soft reset the iOS device. If you don't already know what soft reset is, then press the Home Button and Lock/Power button together for 10 seconds.

However, I can't triple-click. When I try, it just does nothing. Not even every time when I press the home button do I get the Guided Access prompt.
