27 Aug 2014 OBJECTIVE: Asymptomatic primary hyperparathyroidism (PHPT) is routinely encountered in clinical practices of endocrinology throughout the world. This report distills an update of current information about diagnostics, clinical features, and management of this disease into a set of revised guidelines.
New data on the natural history of asymptomatic PHPT have favored surgery because bone density does not appear to be indefinitely stable. Moreover, up to one third of patients who are monitored long term develop signs of disease progression.
See figure: 'Guidelines for surgery in asymptomatic pHPT: a comparison of current recommendations with' from publication 'Declining Rates of Inpatient Parathyroidectomy for Primary Hyperparathyroidism in the US' on ResearchGate, the professional network for scientists.
30 Jun 2017 This has led to a dilemma regarding which patients should be considered for parathyroid surgery. In this article, we review the established literature on the indications for surgery in asymptomatic pHPT and discuss the most recent consensus conference guidelines. OBSERVATIONS The reviewed literature

Accordingly, bone densitometry results suggestive of osteoporosis are logical indications for surgery. Surgery decreases the risk of fracture in PHPT patients by 30% over 20 years. There are two areas not currently included in the formal guidelines: neuropsychiatric and cardiovascular disease.
2 Nov 2014 Asymptomatic PHPT, guidelines for management have been revised. The revised guidelines include: 1) recommendations for more extensive evaluation of the skeletal and renal systems; 2) skeletal and/or renal involvement as determined by further evaluation to become part of the guidelines for surgery;
At this time, there are no prospective data on cardiovascular outcomes in mild PHPT. Moreover, studies that have tracked cardiovascular endpoints before and after surgery have not shown any benefit when the studies have been rigorously
